Form Description
Account Agent Designation statement (also see Designation of Account Agent Policy) Customers must complete the Account Agent Designation Statement to establish an Agency relationship with a third party to allow them to make decisions related to the customer’s account(s).
 
Supplier Authorization form
 Customers must complete the Supplier Authorization form to allow their marketers the authority to nominate on their behalf and to access their account information on the Gas Transport System.
 
Gas Transport Nomination form Customers or marketers use the Gas Transport Nomination form when they do not have access to the Gas Transport System and need to get the nomination submitted in a timely manner.
 
Termination of Account Agent Designation (also see Designation of Account Agent Policy) Customers use the Termination of Account Agent Designation form to end an existing Agency relationship with a third party.
 
Release of Utility Historical Information Customers use the Release of Utility Historical Information form to give Ameren permission to release historical information to a marketer.
